Document Description
The applicant is requesting approval from the City of San Jacinto for a Conditional Use Permit and Site Plan and Design Review to expand the existing 44-bed Bradley Gardens facility to a total of 103 beds on APN 435-330-003. The Project would demolish the existing 1,074 SF maintenance and storage building and construct an approximately 17,372 SF single-story addition to the existing skilled nursing facility (SNF) building providing 43 additional SNF and short-term psychiatric beds, and would convert the existing 2,596 SF vacant office building into a 16-bed psychiatric health facility (PHF). The Project would result in a FAR of 0.27. Additional improvements would include landscaping, utility connections, implementation of stormwater facilities, lighting, parking lot, and driveways.
