Document Description
The Southern Humboldt Community Healthcare District (SHCHD or District), a 501(c) nonprofit organization, is proposing the construction of a new hospital and medical office building (comprising two stories and 49,728 square feet) and a helistop on the properties identified as Assessor’s Parcel Numbers (APNs): 032-091-014, -016, -017, -018, -019, and 032-171-019 (Site), totaling approximately 3.52 acres and generally located at 286 Sprowel Creek Road in the unincorporated community of Garberville in Humboldt County, California. The new hospital and medical office building would replace the existing Jerold Phelps Community Hospital and Southern Humboldt Community Clinic (currently located at 733 Cedar Street and 509 Elm Street) in Garberville. Following completion of the project, the existing Skilled Nursing Unit will remain at the existing hospital facility located on Cedar Street. In addition, renovations are also proposed to an existing building located within the northern portion of the Site.
