            The applicant proposes activities at four stream sites involving Forsythe Creek and an unnamed tributary.  Forsythe Creek is a tributary to the Russian River.  The project will involve the installation of one (one time) temporary culvert crossing, one permanent bridge, a permanent culver placement and a water drafting site.  The California Department of Fish an Game is executing a Lake and Streambed Alteration Agreement R3-200-1335, pursuant to Section 1603 of the Fish and Game Code to the project applicant, Frank Dutra.
